An Advanced Health Care Directive is a legal document authorizing a person to make medical decisions on behalf of the person granting this authority (“Grantor”). Please remember that this document is only effective during the Grantor’s lifetime. This may include authorizing medical treatment under various conditions, communicating with medical personnel, and fulfilling the Grantor’s wishes regarding organ donation.
